Severe acne is characterized by inflammation, formation of cysts, excessive damage to skin and deep scarring. Severe acne requires aggressive acne treatment which needs to taken up under the watchful eyes of dermatologists. Various severe acne treatments which are available today are as follows:

Drainage and surgical excision:
Some cysts are so large and widespread that they do not respond to medication. These have to be physically drained and excised under the watchful eyes of dermatologist. Care should be taken and no attempt is to be made to personally excise them in front of bathroom mirrors. The instruments employed should be properly sterilized. It has been observed in many a cases that patients attempt to cut and puncture these cysts have led to many a problems and in fact have made the condition worse. To drain and surgically excise these cysts, special comedones are used by surgeons.
Interlesional corticosteroid injection
Some of the cysts may become so large that their bursting might create loss of skin tissues and deep scarring. To avoid such situation these cysts are injected with corticosteroid injection which helps is reducing the inflammation and proper healing. After injecting the inflammation subsides and healing start taking place within 3-5 days.
Isotretinoin
It is the most effective treatment for severe acne which has defied all other medications. It is a man made retinoid which comes if form of a pill. Beauty of this pill is that it tries to attack all four factors which are responsible for acne, namely, excess oil production by glands, presence of P. acnes bacteria, clogged pores and inflammation. The course depends upon the severity but normally one or two course of 16-20 weeks is sufficient to address the problem. Although the pill is quite effective in tackling the root problem but it can have many side effects like severe joint pains, nausea, vomiting, thinning of hairs etc. this medication is complete taboo for pregnant women. Because of such strong side effects this medication is administered only in extreme cases.
Oral antibiotics
This is the most popular severe acne treatment available. The treatment works to attack P. acnes bacteria. The treatment begins with stronger than normal dosage and as the results starts showing up, the dosages can be reduced. The popular antibiotics which are being used today are doxycline, erythromycin and tetracycline.
Oral contraceptive
This is one of the very unusual methods of treating acne. It has been observed that contraceptive pills tend to rein in hyper active sebaceous glands. Oral contraceptives can be used as a long term therapy but it should not be used for women who are over 35 years of age. In addition, before its use women has to seek opinion of gynecologists against the likely side effects.br
